Credit Authorization Agreement

Your written authorization for Family First Mutual LLC and its marketplace partners to obtain consumer report information.

August 13, 2026

This Credit Authorization Agreement (“Credit Authorization”) describes the written consent you provide under the federal Fair Credit Reporting Act (“FCRA”), 15 U.S.C. § 1681b(a)(2), when you submit a request through familyfirstmutual.com. Family First Mutual LLC (“Family First Mutual,” “we,” “us,” or “our”) is not a lender, bank, or credit card issuer, and does not make credit decisions.

What You Are Authorizing

By checking the authorization box and clicking “Submit My Request,” you are providing written instructions and consent under the FCRA for Family First Mutual LLC, ML Enterprise Inc. (“Engine by MoneyLion”), and their partners, affiliated companies, subsidiaries, lenders, and financial institution partners to obtain consumer report information from your credit profile from one or more nationwide consumer reporting agencies or other consumer reporting agencies.

You also request that Family First Mutual LLC provide the information you submitted to Engine by MoneyLion and its affiliated companies, subsidiaries, lenders, and financial services partners so that they may provide you with financial recommendations, which may also include debt relief, credit repair, credit monitoring, or other related services.

Permissible Purpose

Your authorization establishes a permissible purpose under the FCRA. Consumer report information will be used solely to:

  • Verify your identity and the accuracy of the information you provided
  • Determine which lending partners and financial products you may be eligible for
  • Present you with personalized offers, recommendations, and rates
  • Prevent fraud and comply with applicable law

Your authorization remains in effect for as long as reasonably necessary to fulfill these purposes, unless and until you revoke it as described below.

Soft Inquiry vs. Hard Inquiry

Soft inquiry. To pre-qualify you and to display estimated offers, our partners typically obtain your consumer report through a soft inquiry. A soft inquiry is visible only to you on your credit report and does not affect your credit score.

Hard inquiry. If you choose to continue with a specific lending partner and complete a full application on that partner’s site, that partner may obtain your consumer report through a hard inquiry. A hard inquiry is visible to other creditors and may affect your credit score. Whether and when a hard inquiry occurs is controlled by the lending partner, not by Family First Mutual LLC.

Offers presented to you are estimates based on the information you provided and are not a commitment to lend. Final terms are determined by the lending partner after verification and underwriting.

How to Revoke This Authorization

You may revoke this Credit Authorization at any time by contacting us using the details below and stating that you are revoking your credit authorization. Please include your full name and the phone number or email address you used when you submitted your request so we can locate your record.

Revocation is effective only after we have had a reasonable opportunity to act on it, and it does not apply retroactively to consumer reports already obtained or to information already shared with a lending partner. A lending partner you have already engaged with may continue to process your application under its own authorization and privacy practices.

Your Rights Under the FCRA

You have the right to obtain a free copy of your credit report from each nationwide consumer reporting agency once every 12 months at annualcreditreport.com. You have the right to dispute incomplete or inaccurate information with the consumer reporting agency and with the furnisher of that information, and the agency must investigate and correct or delete inaccurate information.

If a lending partner takes adverse action based in whole or in part on information in a consumer report, that partner is required to provide you with an adverse action notice identifying the consumer reporting agency it used. Family First Mutual LLC does not make credit decisions and therefore does not issue adverse action notices.

Equal Credit Opportunity Act Notice

The federal Equal Credit Opportunity Act prohibits creditors from discriminating against credit applicants on the basis of race, color, religion, national origin, sex, marital status, age (provided the applicant has the capacity to enter into a binding contract), because all or part of the applicant’s income derives from any public assistance program, or because the applicant has in good faith exercised any right under the Consumer Credit Protection Act. The federal agency that administers compliance with this law concerning this creditor is the Consumer Financial Protection Bureau, 1700 G Street N.W., Washington, DC 20552.
